Is This the Right Procedure for Your Presentation?
The patients I see for this procedure typically have a localised layer of fat around the calves and ankles that is out of proportion with the rest of the leg, and that persists even at a stable weight. The lower leg carries this fat in a circumferential pattern, which can blur the definition between the calf and the ankle. Liposuction (suction assisted lipectomy) removes localised fat.
Skin quality matters here as it does everywhere fat is removed by suction. During consultation, I assess the recoil of the skin over the calf and ankle. Where recoil is good, removing the fat layer allows the skin to settle against the new contour. Where recoil is poor, suction assisted lipectomy alone can leave loose skin, and I will tell you so before any decision is made.
One condition needs specific mention. Bilateral, symmetrical, disproportionate fat of the lower legs, often sparing the feet, and accompanied by heaviness, tenderness or a tendency to bruise, may indicate lipoedema rather than localised fat alone. This is a distinct medical condition and I assess for it at consultation, because it changes both the surgical approach and the expected course. You can read more in my lipoedema guide.
What the Procedure Involves
- Performed as a day procedure at Maitland Private Hospital under general anaesthesia
- Tumescent fluid is infiltrated into the fat layer of the calf and ankle to reduce bleeding and assist fat removal
- VASER (ultrasound assisted lipectomy) energy is applied through a fine probe to loosen the fat before it is removed by suction
- Fat is removed through small incisions placed in the less visible creases of the lower leg
- The calf and ankle are treated circumferentially, working around the leg rather than on one surface only
- The fat volumes removed are smaller than in liposuction (suction assisted lipectomy) of the trunk; the work in this region is about even, precise removal of a thin layer rather than volume
- TED stockings or similar graduated compression stockings are applied before you wake

Risks and recovery
The calf and ankle swell for longer than any other site treated by suction assisted lipectomy. The lower leg sits below the heart for most of the day, fluid pools with gravity, and the final contour can take months to declare itself. Expect visible swelling well beyond the point at which patients treated on the trunk have settled. TED stockings or similar graduated compression stockings are worn full time for the first 4 weeks after surgery and part time for a further 2 weeks, and I encourage walking from the day of surgery with elevation of the legs when seated. The specific risks of this region are prolonged oedema and contour irregularity, which sit alongside the general risks of liposuction (suction assisted lipectomy), including infection, bleeding, altered skin sensation and deep vein thrombosis. Liposuction (suction assisted lipectomy) performed for cosmetic reasons has no Medicare item number, is not covered by private health insurance, and attracts GST; you receive a written quote after your consultation setting out the surgeon, anaesthetist and hospital fees. I examine the calf and ankle, assess your skin recoil, screen for lipoedema, and explain what liposuction (suction assisted lipectomy) can and cannot do for your presentation.
