The referral form your GP completes before a consultation about post weight loss surgery with me.
This is the form I ask GPs to complete before a first consultation. It gives my contact details, the Medicare item numbers that may apply after significant weight loss, and what I check first: at least 5 BMI units lost and weight stable for at least 6 months. Whether surgery suits you is decided at consultation, after a GP referral.
What is inside
- Practice contact details and Healthlink ID for your GP
- MBS item numbers for surgery after significant weight loss (30166, 30177, 30179, 30169)
- Patient criteria your GP records
- Your details and your GP’s details
The referral form is emailed, usually within a few minutes. Check junk if not received. A member of my team may call to confirm it arrived and answer questions about the consultation process. Occasional educational emails follow. Unsubscribe at any time.
