Abdominoplasty Post Pregnancy Patient 2023-1038

A 43-year-old woman, a mother of four children (ages 13, 11, 9, and 2), presents with both aesthetic and functional abdominal concerns. She has had vaginal births for all her children, with no history of C-sections.

The patient reports significant abdominal wall laxity and diastasis recti, characterized by the separation of the abdominal muscles. Despite undergoing physiotherapy, she continues to experience significant back pain and stress incontinence, especially during exercise at the gym.

Her lower abdomen shows visible stretch marks, and there are mild fat deposits in the area. The abdomen appears distended, giving her a “pregnant” appearance, which causes both physical discomfort and emotional distress.

An ultrasound of the abdominal wall confirmed the diagnosis of diastasis recti, with the largest separation located approximately 30mm above the umbilicus, measuring 70mm in maximum transverse dimension. Further divergence above the umbilicus measures between 13 to 50mm, with a more pronounced gap closer to the umbilicus. Inferior to the umbilicus, the separation measures 18mm, and at the level of the umbilicus, it measures 68mm. There is no evidence of abdominal or umbilical hernia upon straining.

The patient is seeking a flatter, more toned abdomen, relief from her chronic discomfort, and improvement in her functional symptoms.

Operation Details:

The patient underwent a full abdominoplasty with VASER liposuction of the abdomen and diastasis recti repair.

  • Fat Removed with liposuction: 1100 ml
  • Abdominal Flap:
    • Right Side: 316g
    • Left Side: 281g
    • Middle Tissue Removed: 153g
  • Diastasis Recti Repair:
    • Midline separation: 8.5 cm (at the umbilicus)
    • Below umbilicus: 9 cm
    • Above umbilicus: 8 cm
  • VASER Liposuction:
    • Total time: 60 minutes
    • Probe used: 5-ringed probe
