30 year old man with functional issues from loose skin after losing 80 kg of weight.
- Lost 80kg of weight
- Lost weight through diet and exercise
- going from 170kg to 90 kg
- Stable weight for a number of years prior to surgery for loose skin
Upon examination, loose skin was observed with minimal breast gland tissue. According to Beldholm’s 2024 gynaecomastia grading system, this is classified as BB grade 4c. The recommended treatment is a Torsoplasty (upper thoracic lift) with free nipple-areolar grafts (NAG).
Operation performed:
- Male Circumferential torsoplasty (upper thoracic lift) with Nipple areolar grafts (NAC)
- Front done initially, excision of excess chest skin. NAC excised and grafted as a free graft onto new position.
- Patient turned side to side
- Circumferential thoracic lift completed on the back.
- Drains where used on both front and back
Tissue removed:
Front on chest:
- R – 728 grams
- L – 809 grams
Back
- R – 538 grams
- L – 433 grams
Total weight removed: 2508 grams or 2.5 kg
