Gynaecomastia Surgery Patient 2022-7010

21 year old male presents with gynaecomastia since he was 14 years old. He has tried to lose weight and go to the gym to minimise his gynaecomastia. However, this has made the abnormal breast gland stand out more.

On examination there is a hard rubbery gland (like a squash ball) under the Areola extending beyond the border of the areola.

According to Dr Beldholm’s gynaecomastia grading system this is a Grade BB2a and the recommended treatment is VASER liposuction with excision of the hard gland.

Operation performed:

• VASER liposuction of the anterior and lateral chest area was performed. Minimal fat was removed.
• Incision made infra-areolar and the excess Breast gland was removed

Histopathology report of removed gland

Macroscopic

1. ‘RIGHT BREAST TISSUE’. – Unoriented fibrofatty breast tissue weighing 12 g and measuring 45 x 46 x 10 mm without attached skin. No lesions identified on inspection or palpation.
2. ‘LEFT BREAST TISSUE’. – Unoriented fibrofatty breast tissue weighing 8 g and measuring 43 x 28 x 19 mm without attached skin. No lesions identified on inspection or palpation.

Microscopic

1. ‘ RIGHT BREAST TISSUE’. – The sections show flbrofatty breast tissue with breast ducts with a fibrous stroma. There is a background of pseudoepitheliomatous stromal hyperplasia. No evidence of dysplasia or malignancy seen. The features are compatible with benign gynaecomastia.
2. ‘LEFT BREAST TISSUE’. – The sections show flbrofatty breast tissue with breast ducts within a fibrous stroma. There is a background of pseudoepitheliomatous stromal hyperplasia. No evidence of dysplasia or malignancy seen. The features are compatible with benign gynaecomastia.
