Gynaecomastia Surgery Patient 2021-7003

Longstanding Gynaecomastia since he was 10 years old.

On examination there was hard breast gland tissue confined to within the Areola. This was making the areola appear “puffy”.

According to Dr Beldholm’s gynaecomastia grading system this is a Grade BB 1. My current recommendation for treatment of this grade is gland excision only with no liposuction. However for this patient I did gland excision with VASER liposuction.

Age:

24 years old at presentation

Operation:

• VASER liposuction performed on anterior chest both sides.
• Incision made under the areola and the breast gland tissue removed on both sides.
• 10F Belovac drains where used and removed the following day.

Histopathology Report

MACROSCOPIC:

Right breast tissue

The specimen consists of a piece of fibroadipose tissue weighing 11g and 52x30x16mm. The outer surface is inked blue. The specimen is sequentially sliced and the cut surface shows fibroadipose tissue with no discrete lesions.

Left breast tissue

The specimen consists of an irregular piece of fibroadipose tissue weighing 14g and measuring 65x36x12mm. The outer surface is inked blue. The specimen is sequentially sliced from one end to the opposite end and the cut surface shows fibroadipose tissue with no discrete lesions.

MICROSCOPIC:

Sections show breast parenchyma with presence of epithelial and stromal changes consistent with florid gynaecomastia in keeping with the radiological impression. There is no evidence of malignancy in the tissue examined.

DIAGNOSIS:

Consistent with Florid gynaecomastia
