This patient has experienced significant gynecomastia since his teenage years. Over time, the gynecomastia has become more pronounced, especially as he has lost weight and built muscle.
During the examination, hard breast gland tissue was noted, extending beyond the areola. The right side was worse than the left, and the right areola was protruding.
According to Dr. Beldholm’s gynecomastia grading system, this condition is classified as Grade BB 2a. My current recommendation for treating this grade is gland excision combined with VASER liposuction, which will be the treatment administered for this patient.

25 years old
Operation:
VASER liposuction was performed on both sides of the chest, including the lateral chest wall. A small incision was made under the areola on both the right and left sides, allowing for the removal of excess breast tissue. A 10F Belovac drain was placed on each side and was removed the following day.
Histopathology Report
MACROSCOPIC:
Right breast tissue
The specimen consists of a piece of fibroadipose breast tissue weighing 25g measuring 64x47x21mm. Cut surface shows fibrofatty tissue, no discrete lesion identified macroscopically.
Left breast tissue
The specimen consists of a piece of fibroadipose breast tissue weighing 17g measuring 59x35x20mm. Cut surface shows fibrofatty tissue, no discrete lesion identified macroscopically.
MICROSCOPIC:
Both specimens contain benign breast parenchyma with no significant fibrocystic change present. The skin present for assessment shows no significant histological abnormality. No atypical intraductal proliferations, in situ or invasive malignancy is identified in the sections examined.
DIAGNOSIS:
Consistent with gynaecomastia
