Gynaecomastia Surgery Patient 2022-7001

“Puffy Nipple” gynaecomastia since starting body building 6 years prior treated with Gland excision and VASER liposuction

Presents with Gynaecomastia for the last 6 years since starting bodybuilding. Puffed out NAC (Nipple areolar complex) obvious to patient in shirts.

On examination has rubbery hard breast gland tissue under the Areola.

According to Dr Beldholm’s gynaecomastia grading system this is a Grade BB 1 and the recommended treatment is excision of the hard gland through an incision under the areola. In this case I performed VASER liposuction as well.

Age:

28 years old at presentation

Operation:

• VASER liposuction performed on anterior chest both sides.
• Incision made under the areola and the breast gland tissue removed on both sides.
• 10F Belovac drains where used and removed the following day.

Histopathology Report

MACROSCOPIC:

Right breast tissue

The specimen consists of a piece of fibroadipose breast tissue without overlying skin measuring 52x50x21mm and weighing 21g. The cut surface shows uniform fibroadipose breast tissue. No focal lesions seen.

Left breast tissue

The specimen consists of a piece of fibroadipose breast tissue without overlying skin measuring 63x48x34mm and weighing 31g. The cut surface shows uniform fibroadipose breast tissue. No focal lesions seen.

MICROSCOPIC:

Sections show breast tissue with numerous ducts including few dilated ducts and patchy apocrine metaplasia. There is no evidence of in situ or invasive malignancy.

DIAGNOSIS:

Consistent with gynaecomastia
