The patient has experienced significant gynecomastia since his teenage years, and this condition has not improved over time. He has noticed that the gynecomastia has become more pronounced since he began bodybuilding.
During the examination, hard breast gland tissue was observed, extending significantly beyond the areola and involving most of the anterior and lateral chest. There is also a small amount of subcutaneous fat present.
According to Dr. Beldholm’s gynecomastia grading system, this condition is classified as Grade BB 3a. My current recommendation for treating this grade is gland excision combined with VASER liposuction and excision of excess skin around the areola. For this patient, I performed gland excision along with VASER liposuction.

19 years old at presentation
Operation:
VASER liposuction was performed on both sides of the chest, including the lateral chest wall. A small incision was made under the areola on both the right and left sides, allowing for the removal of excess breast tissue. A 10F Belovac drain was placed on each side and was removed the following day.
Histopathology Report: 178564-21MP
MACROSCOPIC:
Right gynaecomastia
The specimen consists of a piece of fibroadipose tissue weighing 33g and measuring 60x60x20mm. The cut surface shows fibroadipose tissue and no discrete lesions are seen.
Left gynaecomastia
The specimen consists of a piece of fibroadipose tissue weighing 34g and measuring 65x55x30mm. The cut surface shows fibroadipose tissue and no discrete lesions are seen.
MICROSCOPIC:
Sections of benign breast tissue displaying pseudoangiomatous stromal hyperplasia-like (PASH-like) changes. The section of skin is unremarkable. There is no evidence of in situ or invasive malignancy.
